Transition Network Facilitator (TNF)
The Transition Network Facilitator (TNF) works with school districts and regional agency partners to build strong, collaborative connections. MESD’s TNF serves school districts in Multnomah, Washington, Columbia and Clatsop counties. The TNF shares best practices, strategies, resources and technical assistance with schools and community partners to help improve post-school outcomes. Support is provided to develop policies, procedures and transition planning that support the Governor’s Executive Order. The goal is to increase competitive integrated employment for students and adults in the Portland Metro area to the Oregon coast.
Services offered include:
- Transition in the IEP technical assistance
- Support with General Supervision, Priority 5
- Seamless Transition model TA and ongoing support
- Person Centered Plan (PCP) facilitation & training
- PATH facilitation
- Agency connection support

TNFs are funded through a contract with the Oregon Department of Education. They are a part of the Transition Technical Assistance Network (TTAN), a collective serving the state of Oregon that includes:
- Transition network facilitators (TNFs)
- Vocational Rehabilitation Pre-Employment Transition Services (PRE-ETS) coordinators and providers
- Oregon Developmental Disabilities Services regional liaisons supporting the Employment First Initiative
The TTAN is organized by the Oregon Department of Education (ODE), State of Oregon Vocational Rehabilitation (VR), and the Office of Developmental Disabilities Services (ODDS).
