Have an on-the-job incident or injury to report?
We're sorry to hear about your incident. Here are the tools you need to report your incident.
- ALWAYS tell your Supervisor
- ALWAYS fill out the online Staff Injury Report by clicking the button below to start a Staff Incident Report.
Note: The Staff Incident Report requires you to log in with your MESD email account. If you are unable to enter an Incident Report for any reason, please ask a colleague or supervisor to enter a report for you.
If you have a possible body fluid exposure, ALSO call MESD's Nurse Consultant at (503) 257-1732.
If you see a doctor or other healthcare provider, ALSO download an Injured Workers Packet for your related job site listed in the dropdown below. Fill out the "Worker" section at the top of the included 801 form and send it to MESD HR at workplaceinjury@mesd.k12.
